I found the text for this communion piece in the Lutheran Book of Worship, #220, set to the tune Tender Thought. “O Jesus, blessed Lord, to you my heartfelt praise and thanks are due.” An earnest prayer of thanksgiving for communion distribution, with quiet piano accompaniment and a second verse which can be sung a cappella.
